April Brinkhoff, MD, FAAFP – ACO Board Chair

Fallbrook Family Health Center

Dr. Brinkhoff is a physician at Fallbrook Family Health Center. She obtained a bachelors degree from of the University of Nebraska-Lincoln and received her medical degree from the University of Nebraska Medical Center in Omaha. She then completed her residency at the Lincoln Family Medicine Program. Dr. Brinkhoff is board certified in Family Medicine and is a member of the American Academy of Family Practice.

Rachel Blake, MD, FAAFP – ACO Board Vice-Chair

Primary Care Partners

Dr. Blake is an owner and partner at Primary Care Partners. She is a graduate of Chadron State College and the University of Nebraska Medical Center College of Medicine. Dr. Blake is a veteran of Operation Iraqi Freedom and served for 13 years with the Nebraska Army National Guard. She is Board Certified in Family Medicine and is a Fellow of the American Academy of Family Physicians.

Thomas Fischer, MD

Family Physicians Group

Dr. Fischer is a physician at Family Physicians Group. He graduated from the University of South Dakota and received his medical degree from the University of South Dakota Sanford School of Medicine. Dr. Fischer then completed his residency at the Lincoln Medical Education Program here in Lincoln.

 Sian Jones-Jobst, MD, FAAP

Complete Children’s Health, PC

Dr. Jones-Jobst is a physician at Complete Children’s Health. Originally from Scotland, Dr. Jones-Jobst emigrated to the United States in 1974. She received her bachelors degree from The John Hopkins University, School of Arts and Sciences, Baltimore Maryland and obtained her medical degree from the University of California, San Diego. She completed her residency training in General Pediatrics at the Creighton – Nebraska Health Foundation. Dr. Jones-Jobst is board certified by the American Board of Pediatrics. 

Robert Koch, MD, FAAP

Children First Pediatrics, PC

Dr. Koch is the founder and owner at Children First Pediatrics. He attended Creighton University and the University of Nebraska Medical Center. Dr. Koch completed his pediatric internship and residency with the U.S. Army in Honolulu, Hawaii. He serves the community by sitting on various medical boards. 

Todd Martin, MD

Women’s Health Care Center of Williamsburg

Dr. Martin is a physician at Women’s Health Care Center of Williamsburg. He obtained his bachelor’s degree at Creighton University, his masters in chemistry at the University of Colorado, and returned to Creighton for medical school and residency. He is Board Certified in OB GYN and focuses on both routine and high risk obstetrics.

Jason Potts, MD

Lincoln Internal Medicine Associates

Dr. Potts is a physician at Lincoln Internal Medicine Associates. He graduated from the University of Nebraska Medical Center and completed his residency in Internal Medicine-Primary Care at the University of Nebraska Medical Center. Dr. Potts is Board Certified in Internal Medicine.

Diane Wilson, MBA, CPA

Medicare Representative

Wilson worked in finance for the majority of her professional career. She received her bachelors degree from the University of Iowa where she was a member of the women’s varsity basketball team. She then went on to complete her masters of business administration at the John E. Anderson Graduate School of Management, UCLA.
